    Pseudophakic cystoid macular edema: update 2016

    Pseudophakic cystoid macular edema (PCME) is the most common complication of cataract surgery, leading in some cases to a decrease in vision. Although the pathogenesis of PCME is not completely understood, the contribution of postsurgical inflammation is generally accepted. Consequently, anti-inflammatory medicines, including steroids and nonsteroidal anti-inflammatory drugs, have been postulated as having a role in both the prophylaxis and treatment of PCME. However, the lack of a uniformly accepted PCME definition, conflicting data on some risk factors, and the scarcity of studies comparing the role of nonsteroidal anti-inflammatory drugs to steroids in PCME prevention make the problem of PCME one of the puzzles of ophthalmology. This paper presents an updated review on the pathogenesis, risk factors, and use of anti-inflammatory drugs in PCME that reflect current research and practice

    Serum biomarkers and risk of hepatocellular carcinoma recurrence after liver transplantation

    Liver transplantation (LT) is the only potentially curative treatment for selected patients with cirrhosis and hepatocellular carcinoma (HCC) who are not candidates for resection. When the Milan criteria are strictly applied, 75% to 85%of 3- to 4-year actuarial survival rates are achieved, but up to 20% of the patients experience HCC recurrence after transplantation. The Milan criteria are based on the preoperative tumor macromorphology, tumor size and number on computed tomography or magnetic resonance imaging that neither correlate well with posttransplant histological study of the liver explant nor accurately predict HCC recurrence after LT, since they do not include objective measures of tumor biology. Preoperative biological markers, including alpha-fetoprotein, desgamma- carboxiprothrombin or neutrophil-to-lymphocyte ratio and platelet-tolymphocyte ratio, can predict the risk for HCC recurrence after transplantation. These biomarkers have been proposed as surrogate markers of tumor differentiation and vascular invasion, with varied risk magnitudes depending on the defined cutoffs. Different studies have shown that the combination of one or several biomarkers integrated into prognostic models predict the risk of HCC recurrence after LT more accurately than Milan criteria alone.     System Orchestration Support for a Collaborative Blended Learning Flow

    Portable and interactive technologies are changing the nature of collaborative learning practices and open up new possibilities for Computer Supported Collaborative Learning (CSCL). Now, activities occurring in and beyond the classroom can be combined and integrated leading to a new type of complex collaborative blended learning scenarios. However, to organize and structure these scenarios is challenging and represent a workload for practitioners, which hinder the adoption of these technology-enhanced practices. As an approach to alleviate this workload, this paper proposes a proof of concept of a technological solution to overcome the limitations detected in an analysis of an actual collaborative blended learning experiment carried out in a previous study. The solution consists on a Unit of Learning suitable to be instantiated with IMS Learning Design and complemented by a GenericService Integration system.     The ongoing volcanic eruption of El Hierro, Canary Islands

    El Hierro, the youngest of the Canary Islands (Spain), is no stranger to hazards associated with volcanic activity or to efforts to minimize the effects of these hazards on local communities. As early as 1793, administrative records of El Hierro indicate that a swarm of earthquakes was felt by locals; fearing a greater volcanic catastrophe, the first evacuation plan of an entire island in the history of the Canaries was prepared. The 1793 eruption was probably submarine with no appreciable consequences other than that the earthquakes were felt [Carracedo, 2008]; over the next roughly 215 years the island was seismically quiet. Yet seismic and volcanic activity are expected on this youngest Canary Island due to its being directly above the presumed location of the Canary Island hot spot, a mantle plume that feeds upwelling magma just under the surface, similar to the Hawaiian Islands. Because of this known geologic activity, the Spanish Instituto Geogrfco Nacional (IGN) has managed geophysical monitoring of the island since the beginning of the 1990s.Peer Reviewe

    Development and validation of a questionnaire to evaluate lifestyle-related behaviors in elementary school children

    Background The SI! Program promotes cardiovascular health through a multilevel school-based intervention on four lifestyle-related components: diet, physical activity, understanding the body and heart, and management of emotions. We report here the development and validation of the KAH (knowledge, attitudes and habits)-questionnaire adapted for elementary school children (6-7 years old) as a tool for the forthcoming evaluation of the SI! Program, where the KAH scoring will be the primary outcome. The efficacy of such an intervention will be based on the improvements in children's KAH towards a healthy lifestyle. Methods The questionnaire validation process started with a pool of items proposed by the pedagogical team who developed the SI! Program for elementary school. The questionnaire was finalized by decreasing the number of items from 155 to 48 using expert panels and statistical tests on the responses from 384 children (ages 6-7). A team of specialized psychologists administered the questionnaire at schools providing standard directions for the final administration. The internal consistency was assessed using Cronbach's α coefficients. Reliability was measured through the split-half method, and problematic items were detected applying the item response theory. Analysis of variance and Tukey's test of additivity were used for multiple comparisons. Results The final KAH-questionnaire for elementary school children should be administered to children individually by trained staff. The 48 items-questionnaire is divided evenly between the 4 components of the intervention, with an overall Cronbach's α = 0.791 (α = 0.526 for diet, α = 0.537 for physical activity, α = 0.523 for human body and heart, and α = 0.537 for management of emotions). Conclusions The KAH-questionnaire is a reliable instrument to assess the efficacy of the SI! Program on instilling healthy lifestyle-related behaviors in elementary school children

    Uncertainties in the determination of cross-link density by equilibrium swelling experiments in natural rubber

    ABSTRACT: Equilibrium swelling is a feasible and simple experiment to determine the cross-link density of networks. It is the most popular and useful approach; however, in most of the cases, the given values are highly uncertain if not erroneous. The description of the complex thermodynamics of swollen polymer networks is usually based on the Flory-Rehner model. However, experimental evidence has shown that both the mixing term described by the Flory-Huggins expression and the elastic component derived from the affine model are only approximations that fail in the description and prediction of the rubber network behavior. This means that the Flory-Rehner treatment can only give a qualitative evaluation of cross-link density because of its strong dependence on the thermodynamic model. In this work, the uncertainties in the determination of the cross-link density in rubber materials by swelling experiments based on this model are reviewed. The implications and the validity of some of the used approximations as well as their influence in the relationship of the cross-link densities derived from swelling experiments are discussed. Importantly, swelling results are compared with results of a completely independent determination of the cross-link density by proton multiple-quantum NMR, and the correlation observed between the two methods can help to validate the thermodynamic model

    Handling Conflicts in Depth-First Search for LTL Tableau to Debug Compliance Based Languages

    Providing adequate tools to tackle the problem of inconsistent compliance rules is a critical research topic. This problem is of paramount importance to achieve automatic support for early declarative design and to support evolution of rules in contract-based or service-based systems. In this paper we investigate the problem of extracting temporal unsatisfiable cores in order to detect the inconsistent part of a specification. We extend conflict-driven SAT-solver to provide a new conflict-driven depth-first-search solver for temporal logic.
